Lokeswara Rao Sajja, MS., MCh, FACS (USA)

Personal Allusion

I am humbled to be a Senior Consultant Cardiac Surgeon in the Department of Cardiothoracic Surgery at STAR Hospitals, Hyderabad, India. Throughout my academic career, I have been lucky enough to train with the giants of coronary artery bypass surgery. While bypass surgery may appear as one of the more basic cardiac operation, the complexities of the patient and the variations of the operation require exquisite attention to detail to keep patients safe, which is my main priority. With the support of the incredible staff, including the cardiac anesthesia team, nurses, technicians and surgeons, I have the utmost confidence that I am in the best possible environment to provide the highest level of care to my patients.

Biography

Dr. Lokeswara Rao Sajja is a Senior Consultant Cardiothoracic Surgeon and In-charge of Academics, Director of Cardiovascular Research at STAR Hospitals, Hyderabad, India.

Dr. Sajja completed his undergraduate studies and earned his medical degree in Guntur, India at the Guntur Medical College. He subsequently completed his MS at the JIPMER, Pondicherry, and also completed his super-specialization in Cardiothoracic and Vascular Surgery at the AIIMS, New Delhi.   He obtained advanced training under world-renowned Cardiac Surgeon, Prof. Denton A. Cooley at the Texas Heart Institute, Houston, Texas, USA.

In addition to his clinical expertise, Dr. Sajja is currently the In-charge of Academics and Director of Research in the Department of Cardiothoracic Surgery. Throughout his academic career, Dr. Sajja has authored more than 75 peer-reviewed journal articles. He was the lead investigator of an Indian, multi-centric randomized, controlled trial, the first multi-centric RCT in Cardiac Surgery in India, which aimed to determine the optimal strategy for coronary artery bypass surgery (the PROMOTE Patency Trial). He also conducted mono-centric RCT in Cardiac Surgery to determine the optimal strategy for CABG in patients with renal insufficiency.  He is also an Investigator in Arterial Revascularization Trial, the largest multi-national RCT in Cardiac Surgery, lead center at Oxford. He has given expert presentations at national and international meetings to share his knowledge of coronary artery bypass surgery and the use of multiple arterial grafts. He currently serves as the President of the Society of Coronary Surgeons and also as the Vice-President of the Indian Association of Cardiovascular-Thoracic Surgeons.

Dr. Sajja's interests include all aspects of adult cardiac surgery. Along with his expert training and knowledge in coronary artery bypass surgery, including the use of multiple arterial grafting (radial artery conduits, the use of bilateral internal mammary arteries), Dr. Sajja also specializes in structural heart disease, valvular dysfunction, and complex aortic procedures. He designed a mechanical prosthetic valve that does not require long-term anticoagulation, for which he was granted patency by the United States Patent and Trademark Office.

As a Chairman of Sajja Heart Foundation, Dr. Sajja oversees ongoing clinical trials and provides strategic growth plans to increase investigator-initiated trials, pragmatic trials, and ongoing improvements and enhancements to the existing clinical trial infrastructure.

Clinical Trials Experience

(i)            He is a keen researcher and a master clinical trialist in Cardiac Surgery. He conducted India's first multicentre Randomized Control Trial (RCT) in Cardiac Surgery (PROMOTE Patency Trial).

(ii)         He has represented India as a Site Principal / Co- Investigator in multinational clinical Trials in Cardiac Surgery (Arterial Revascularization Trial, ROMA Trial).

Coronary Artery Bypass Graft (CABG)

Increased prevalence of CAD in India has led to an increase in number of coronary artery bypass grafting. CABG accounts for more than 60% of all cardiac operations and every year more than 1, 50,000 coronary bypass operations are being carried out in India.

The STAR Hospitals Department of Cardiothoracic Surgery has long history of performing coronary artery bypass surgery; the most commonly performed surgical procedure for treating multi-vessel coronary artery disease.  The mortality rate among these patients was less than 1%.  Our outcomes are consistently among the best in India, and routinely exceed national average and match the best centers in the US.

The coronary artery disease affects many of the younger individuals at the prime of their productive life, about a decade earlier than our western counterparts. The most common surgical treatment for coronary artery disease is coronary artery bypass graft (CABG). This procedure has proven to lower the risk of future heart attack and the need for additional procedures. It has also proven to result in a longer life expectancy.

Since many of the recent trials were not yet published at time of writing the guidelines, the KDIGO Work Group concluded that there was not enough evidence to recommend off-pump coronary artery bypass for reducing AKI or the need for RRT (Kidney Disease: Improving Global Outcomes (KDIGO) Acute Kidney Injury Work Group, 2012). Based on the study by Sajja et al. (2007), this opinion should probably be modified for patients with advanced CKD.
